Birmingham Hip Resurfacing (BHR) System
Hundreds of thousands of hip replacement surgeries are performed every year in the United States, and hip resurfacing is one form of the procedure. For those who suffer substantial hip pain due to degenerative joint disease, this procedure has the potential to increase their mobility and quality of life. While it offers a more conservative approach than a total hip replacement, hip resurfacing surgery still involves a risk of complications and a lengthy recovery period.

The Birmingham Hip Resurfacing System (BHR) was originally approved by the FDA for use in the United States in 2006. Developed by Smith & Nephew, this system was designed to allow for more conservation of the bone as compared to a total hip replacement. Instead of removing all of the bone within the hip joint, the BHR system only requires that a few centimeters of bone are resurfaced. This is done by a surgeon placing a metal cap on the femur head which then pivots in an implanted metal cup within the pelvis.
While the design of the BHR System was intended to allow younger patients to enjoy a more active lifestyle with fewer surgical complications, metal on metal hip devices have faced numerous issues over the years. Failure rates for hip resurfacing implants are high, and one study showed that 8.3% of these implants fail within five years in women over age 55.

Concerns with the BHR System
Due to the high failure rate in women, researchers recommended that only men consider having this device used in their hip replacement surgery. In addition to the concerns over failure rates, there are several other risks associated with the use of the metal on metal BHR System, including the following:
• Chronic joint and groin pain
• Metallosis (metal poisoning)
• Bone loss
• Death of tissue surrounding the joint
• Fracture
• Decreased mobility
In many patients with a metal on metal hip system, revision surgery becomes necessary in the future. This type of surgery is often much more complex than the original hip replacement procedure, and patients face an increased risk of further complications.

In response to the alarming statistics of device failure and metal poisoning in patients with the BHR System, Smith & Nephew issued a safety warning to the public in 2012. At this time, they also recalled the metal liner used in the device. From the time of the BHR System’s release through the recall, over 7,700 patients had received the metal liner associated with the increased risk of complications. To date, numerous lawsuits have been filed against the company by those who received the metal on metal version of the BHR System.
